This August we're taking 10 lucky young people with heart conditions to Portugal (applications for places are now closed). The trip is an opportunity to:
Meet and get to know other teenage heart patients
Make your voice REALLY heard, by joining a new teen forum
Experience new adventures with lots of fun planned activities
